West Kent CareCall is a free telephone support service that was introduced to complement local GP and

to encourage patients, particularly those with long term conditions, to take more control of their health.

This is achieved through the use of specially trained health coaches who promote greater awareness of how conditions can be managed at home and the importance of issues such as medication compliance in controlling conditions such as diabetes and heart conditions.

Working with the PCT and Bupa Health Dialog, Gillian managed the communications roll out to GP practitioners and to the wider healthcare community, and the subsequent patient engagement programme to promote the benefits of the programme.  To date a quarter of a million patients are registered for the programme and thousands of patients throughout West Kent are benefiting from the enhancement to their healthcare service.
